What to do when you notice that you can’t find your Smart Serve Certification card – don’t panic! If you are certified, your training status currently does not expire with Smart Serve Ontario. So, even if it has been a few year’s since you took the training course, you are not alone in needing to order a replacement card. If you need a new card, it will arrive in the mail, usually within a few weeks of ordering online. In the meantime, you can always see your Certification number in your online account profile – Login Now.
How do I order a new Smart Serve Certification card?
A replacement card will cost $10.00 and can be ordered from your Individual online account. If you don’t have an online profile (because you did a paper test or completed online training prior to 2011) you will need to register for one! When you create your online profile, it will LINK to your current Certificate number in our database, based on your legal name and date-of-birth.
- Go to smartserve.ca
- Register for an – Individual Online Account
- Ensure your personal contact information matches your I.D.
- Complete all validation steps (check your junk box!)
- Your account will then be linked and you can proceed with placing your order for a replacement card and confirming your shipping address
Don’t Forget! Your online account profile will be linked based on your full legal name and date of birth. If you have changed your name or the spelling that was on your original certification card, you will be required to complete an additional verification step and provide proof of identification to our office.
